Job summary

Comprehensive Rehab Consultants (CRC) seeks a NP/PA PM&R Clinician to deliver specialized physiatry services in designated SNFs and post-acute rehab centers. You will lead clinical care, collaborate with interdisciplinary teams, and promote patient independence while maintaining flexible, predictable hours.

Qualifications

Current NP/PA license in Washington state (WA).
Post-acute/SNF experience preferred.
Ability to work autonomously and manage patient care in a facility setting.
US work authorization to practice in the United States.

Responsibilities

Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
Represent CRC in the facility with clinical leadership and interdisciplinary collaboration.
Collaborate with PCP teams to manage conditions common in SNFs (e.g., CVA, neurological conditions, postoperative ortho).
Coordinate with PT/OT/SLP teams to improve function and quality of life.
Monitor patient progress and adjust therapy as needed; track therapy outcomes.
Attend multidisciplinary meetings and bridge therapy and internal medicine teams.
Participate in culture building, education sessions, and regional meetings.
