Job summary

Comprehensive Rehab Consultants is seeking a part-time PM&R Clinician (Nurse Practitioner/ APRN) for Edinboro, PA. The clinician will provide specialized PM&R services at a designated SNF, collaborating with the post-acute therapy team to optimize rehabilitative outcomes and patient independence.

Role emphasizes autonomy with no evening or on-call requirements, access to a scribe and remote support, and opportunities for certification and ongoing education.

Qualifications

Nurse Practitioner/ APRN license required in PA.
Post-acute/SNF experience preferred.
Ability to work autonomously and collaborate with interdisciplinary team.

Responsibilities

Provide PM&R/Physiatry services in a SNF or post-acute rehab center.
Represent CRC in facility with clinical leadership and collaboration with the care team.
Coordinate with PCPs to manage conditions common in SNFs (e.g., CVA, progressive neuro conditions, postoperative ortho).
Work with PT/OT/SLP teams to improve patient function and quality of life.
Monitor patient progress and follow therapy to track outcomes.
Attend multidisciplinary meetings and contribute to culture-building activities.
