Comprehensive Rehab Consultants in Edinboro, PA, is seeking a PM&R Clinician (APRN) to provide Physiatry services exclusively at a designated SNF or post-acute rehab center. You will lead in collaboration with the post-acute therapy team to optimize rehabilitation outcomes and promote patient independence.
The ideal candidate is licensed as an APRN/NP in PA, has post-acute/SNF experience, and can work autonomously with strong clinical leadership.
Competenze
- Nurse Practitioner/APRN license and Pennsylvania license required.
- Ability to work in the U.S.
- Post-acute/SNF experience preferred.
- Autonomous practice with strong communication.
- Compassionate and patient-centered care.
Mansioni
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility with clinical leadership.
- Collaborate with PCP teams for SNF conditions (CVA, neuro, post-op).
- Work with PT/OT/SLP teams to improve function and life quality.
- Follow therapy patients to track progress.
- Assess and manage barriers to therapy (pain, spasticity, neuro issues).
- Attend multidisciplinary meetings and bridge therapy with internal medicine.
- Engage in culture-building and ongoing education (Didactics, regional meetings).
