Job summary

Comprehensive Rehab Consultants seeks a Nurse Practitioner/APRN/Physician Assistant to provide PM&R/Physiatry services in SNF/post-acute settings. The clinician will work with a multidisciplinary team to optimize rehabilitation outcomes and patient independence.

The ideal candidate demonstrates autonomy, strong interpersonal skills, and the ability to lead clinically while collaborating with PCP, PT, OT, and SLP teams within CRC’s growing post-acute care network.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage conditions common in SNFs such as CVA and postoperative orthopedic surgeries.
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress and address barriers to therapy.
