Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner/PA to provide PM&R services at designated SNF/post-acute centers in Lake City, FL. The clinician will work with a multidisciplinary team to optimize rehabilitative outcomes and promote patient independence.

• Post-acute experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with PCPs and interdisciplinary teams to manage SNF/post-acute patients.
• Support therapy colleagues to improve patient function and quality of life.
• Attend multidisciplinary meetings and contribute to culture building.
