Job summary

Comprehensive Rehab Consultants (CRC) seeks a NP/PA to provide PM&R/Physiatry services at a designated SNF or post-acute center in Texas City, TX. You will collaborate with an interdisciplinary team and PCPs to optimize patient outcomes.

The role emphasizes autonomous practice with a focus on geriatric rehabilitation, workflow balance, and ongoing education, including certifications and training opportunities provided by CRC.

Qualifications

• Nurse Practitioner or PA license valid in Texas.
• Experience in post-acute/SNF care preferred.
• Ability to work autonomously in a post-acute setting.

Responsibilities

• Provide PM&R/Physiatry services at a designated SNF or post-acute center.
• Collaborate with interdisciplinary care teams and PCPs to manage conditions common in SNFs.
• Support therapy teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
