Job summary
Comprehensive Rehab Consultants is seeking a Nurse Practitioner/PA to provide PM&R/Physiatry services at designated SNF/post-acute rehab centers in Texas. You will lead clinical care, collaborate with interdisciplinary teams, and drive patient outcomes in geriatric populations.

Qualifications
- Active NP/ARNP/PA license in Texas.
- Experience in post-acute SNF/rehab preferred.
- Ability to work in the U.S.
- Strong interpersonal skills and empathy.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
- Collaborate with PCP teams to manage SNF conditions such as CVA and progressive neurological conditions.
- Work with PT, OT, and SLP teams to improve patient function and quality of life.
- Follow therapy patients to track progress in therapy.
- Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
- Attend multidisciplinary meetings and bridge therapy with internal medicine.
- Engage in culture building and ongoing education including monthly lectures and regional events.
