Job summary
Comprehensive Rehab Consultant in Massachusetts is seeking a part-time PM&R clinician to provide specialized Physical Medicine and Rehabilitation in SNF settings. You will lead clinical care in collaboration with an interdisciplinary team to improve patient function and quality of life.

Qualifications
- Licensed NP/APRN/PA with ability to practice in Massachusetts.
- Experience in post-acute or SNF settings preferred.
- Ability to work autonomously and manage independent caseload.
- Strong interpersonal and collaboration with interdisciplinary teams.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC with strong clinical leadership in facilities.
- Collaborate with PCP teams for SNF conditions.
- Coordinate with PT/OT/SLP to improve patient function.
- Track patient progress and address therapy barriers.
- Attend multidisciplinary meetings and support culture building.
