Comprehensive Rehab Consultants in Providence/Bristol, RI seeks a Nurse Practitioner or PA to provide PM&R/Physiatry services in a designated SNF or post-acute rehab setting. You will lead clinical care within the interdisciplinary team and work autonomously with a consistent daily schedule.

Qualifications

• SNF or post-acute experience preferred.
• Rhode Island license required.
• Able to work in the U.S. without sponsorship.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with the interdisciplinary care team at the SNF.
• Coordinate with PCPs to manage post-acute conditions.
• Work with PT/OT/SLP to improve patient function.
• Attend multidisciplinary meetings and support culture building.
