Job summary

Comprehensive Rehab Consultants in Providence/Bristol, RI is seeking a Nurse Practitioner/PA to join as a PM&R clinician in a 40-hour, full-time W-2 role at designated SNFs. You will work with post-acute therapy teams to optimize rehabilitation outcomes and promote patient independence.

The ideal candidate has NP/PA credentials, RI license, and experience in post-acute care. Excellent collaboration skills and a commitment to high-quality geriatric care are essential.

Qualifications

• Nurse Practitioner/APRN/PA credentials required.
• Rhode Island license required.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide PM&R services at SNF/post-acute centers.
• Represent CRC in your facility with an interdisciplinary care team.
• Collaborate with PCPs and therapy teams to manage SNF conditions.
• Work with PT/OT/SLP teams to improve patient function.
• Follow therapy patients to track progress in therapy.
• Assess and address barriers to therapy (pain, spasticity, neuro deficits).
• Attend multidisciplinary meetings and support ongoing education.
