Job summary

Concord Hospital Health System is seeking a Patient Care Coordinator to support patient registration, referral coordination and phone management. You will be the primary access point for patients and families and ensure timely, efficient, and compassionate customer service.

The role requires a high school diploma and demonstrates strong customer service and knowledge of medical office operations, including scheduling, EMR, billing and referrals.

Qualifications

• High School degree or GED required.
• Proven customer service experience.
• Knowledge of medical office operations including scheduling, registration, EMR, patient flow, billing, coding, referrals and compliance.

Responsibilities

• Greets and assists patients; secures signatures; follows CHMG policies.
• Obtains and verifies insurance and demographic information; makes collection attempts; reconciles work lists.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information and maintains medical records.
• Processes and routes calls and responds to inquiries.
• Schedules, confirms or reschedules patient appointments; promotes Patient Connect enrollment.
• Distributes mail, faxes and electronic documents; educates patients on form completion.
• Supports a culture of “yes” and works independently or in a team on projects.
