SHIELDS for Families in Los Angeles is seeking a Navigator for the Plans of Safe Care (POSC) program at Olive View Hospital. The role provides prenatal and postpartum support, conducts assessments, enrolls participants, and collaborates with families and hospital staff.

You will educate on care, coordinate services, document records, and connect families with behavioral health, substance use treatment, and community resources.

Qualifications

• Candidates should have basic advocacy skills and the ability to clearly communicate ideas both verbally and in writing.
• Strong organizational, analytical, and computer skills including proficiency with word processing software.
• Experience working with individuals with substance use disorders and underserved populations; bilingual Spanish preferred.
• Bachelor’s degree preferred; relevant experience may substitute for education.
• Valid California driver’s license, car insurance, driving record, and reliable transportation.

Responsabilités

• Assess and enroll participants at the bedside; develop POSC with families.
• Conduct prenatal and postpartum home visits; provide parenting and maternal health education.
• Coordinate referrals to services and community resources.
• Maintain accurate documentation and POSC records.
• Collaborate with DCFS and other referral sources.
• Submit monthly updates to the program manager and attend trainings.
