Job summary

Arizona Youth Partnership seeks a Health Navigator to provide individualized support and care coordination for participants, focusing on pregnancy, parenting, recovery goals, and access to healthcare and social services.

You will conduct screenings, develop personalized plans, and connect participants with MOUD/MAT and behavioral health services, while maintaining progress with follow-ups and reporting obligations.

Qualifications

• High school diploma or GED required.
• Excellent presentation, oral and written communication skills.
• Knowledge of Word, Excel, Publisher, Power Point a plus.

Responsibilities

• Provide individualized support, encouragement, advocacy, and care coordination.
• Assist participants in identifying needs and developing wellness goals.
• Coordinate services across healthcare, social services, and MOUD/MAT resources.
• Maintain regular contact with participants and monitor progress.
• Complete required program performance measures and reporting.
• Represent AZYP at public events to promote prevention efforts.
