Chestnut Health is seeking a Physician Assistant to join its physiatry team in a patient-centered, collaborative environment within skilled nursing facilities. You will help develop and implement comprehensive rehab plans, perform evaluations, monitor progress, and support families through the recovery process.

Qualifications

• Actively Licensed to practice in the state of employment
• Current Controlled Substance License (required on start)
• Current DEA License (required on start)
• NCCPA Certificate
• Desire to work in Physiatry/PM&R
• Orthopedic Experience (nice to have)
• Physiatry Experience (nice to have)
• Skilled Nursing Experience (nice to have)

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records
• Provide education and counseling to patients and families regarding rehabilitation process, expected outcomes, and post-discharge care
