Chestnut Health Company is seeking a Physician Assistant to join our physiatry team in a collaborative, patient-centered environment focused on rehabilitation in Skilled Nursing Facilities. You will contribute to comprehensive treatment plans and help patients regain function and independence.

You will conduct assessments, support multidisciplinary teams, document evaluations and progress, and educate patients and families about the rehabilitation process and post-discharge care.

Formación

• Active license to practice in the state of employment.
• Current Controlled Substance License required upon start date.
• Current DEA License required upon start date.
• NCCPA Certification.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Responsabilidades

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including histories, exams, and evaluations to set rehab goals and interventions.
• Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Educate patients and families about the rehabilitation process, outcomes, and post-discharge care.
