Chestnut Health is seeking a Physician Assistant to join our physiatry team, delivering patient-centered rehabilitation care in collaboration with therapists and nursing staff. You will assess patients, develop plans, and monitor progress to promote independence in functional outcomes.

Competenze

• Actively licensed to practice in the state of employment.
• Current Controlled Substance License required on start date.
• Current DEA License required on start date.
• NCCPA Certification.
• Desire to work in Physiatry/Physical Medicine and Rehabilitation.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Mansioni

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
• Conduct patient assessments to determine rehabilitation goals and interventions.
• Monitor progress and adjust treatment plans to optimize outcomes and independence.
• Participate in multidisciplinary meetings to enhance clinical knowledge.
• Maintain timely EMR documentation including evaluations, plans, progress notes, and discharge summaries.
• Provide education to patients and families about rehabilitation and post-discharge care.
