Job summary

Chestnut Health seeks a devoted Physician Assistant to join the physiatry team working in Skilled Nursing Facilities. You will contribute to patient-centered rehabilitation aimed at restoring function and independence.

You will collaborate with therapists and nurses, perform comprehensive assessments, guide care plans, and document care in the EMR. A strong interest in PM&R and current licensure are required, with opportunities to grow in a supportive setting.

Qualifications

• Actively licensed to practice in the state of employment.
• Current DEA license required on start date.
• Current Controlled Substance license required on start date.
• NCCPA certification is required.
• Desire to work in Physiatry/PM&R.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to implement rehab plans.
• Conduct patient assessments including histories, examinations, and evaluations.
• Monitor progress and adjust plans to maximize function and independence.
• Attend multidisciplinary meetings and contribute to clinical knowledge.
• Document evaluations, plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehabilitation and post-discharge care.
