Chestnut Health is seeking a Physician Assistant to join our physiatry team dedicated to rehabilitation in skilled nursing facilities. You will work in a collaborative, patient-centered environment where your expertise directly affects functional outcomes and independence.

You will collaborate with therapists and nurses to develop comprehensive plans, assess patients, monitor progress, and maintain thorough documentation in EMR. A focus on education for patients and families is essential.

Qualifications

• Licensed to practice in the state where employed.
• Current Controlled Substance license required upon start date.
• Current DEA license required upon start date.
• NCCPA certification present.
• Interest in Physiatry/PM&R.

Responsabilités

• Collaborate with rehab therapists, nurses, and other professionals to develop and implement comprehensive treatment plans for rehabilitation patients.
• Conduct patient assessments including histories, exams, and evaluations to determine rehab goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings, case conferences, and continuing education activities.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding rehab process and post-discharge care.
