Job summary

Chestnut Health in the United States seeks a Physician Assistant to join our physiatry team in SNF settings. You will collaborate with therapists, nurses, and physicians to develop rehab plans and monitor progress to improve function and independence.

New graduates are encouraged to apply. Chestnut Health requires active Indiana licensure, NCCPA certification, and a current DEA license on start date; orthopedic or SNF experience is a plus.

Qualifications

• Active licensure in Indiana.
• NCCPA certification is required.
• Current DEA license is required on start date.
• Desire to work in Physiatry/Physical Medicine and Rehabilitation.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct patient assessments, including medical histories, physical exams, and diagnostic evaluations, to determine rehabilitation goals.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding rehabilitation process and post-discharge care.
