Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) for a Skilled Nursing Facility in the NYC area. You will work with a multidisciplinary team to deliver treatments and patient education to improve function and cognition, following the established Plan of Care.

We offer options for Direct Employment or to Be Your Own Boss as an Independent Contractor, with weekly pay and comprehensive benefits. NYS PTA license and EMR competence are required.

Qualifications

• Current NYS PTA License and Registration.
• EMR competency required.
• Meets all State and Federal requirements for US employment in a Nursing Facility.

Responsibilities

• Collaborate with PT Supervisor and follow Plan of Care for all patients.
• Provide quality therapy services to SNF residents with diverse diagnoses.
• Monitor and document patient progress using EMR.

Physical Therapy Assistant (PTA) in Skilled Nursing Facility

Now Staffing Physical Therapist Assistants for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

PTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

We offer the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Job Duties:

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
