Job summary

Flagstar Rehab is seeking a Physical Therapist (PT) to join our skilled nursing facility teams in NYC and surrounding areas. You will evaluate patients, develop individualized treatment plans, and educate patients and families while collaborating with a diverse, multi-disciplinary team.

We offer flexible schedules (FT/PT/Per Diem), weekly pay, paid orientation, and optional insurance. A NYS PT license and CPR are required, and EMR competence is expected.

Qualifications

• Current NYS PT License and Registration.
• CPR Certification.
• EMR Competence.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in a SNF setting, addressing conditions such as TBI, CVA, stroke, Parkinson’s, MS, cancer, and orthopedic surgery.
• Monitor and document patient progress with EMR.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor. We can assist you with setting up a corporation free-of-charge.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
