Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Tampa, Florida. You will work under the supervision of a Physical Therapist to deliver resident-centered therapy services and help maintain quality care.

The role emphasizes screenings, treatment planning, group and individual therapy, and accurate CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate’s degree in physical therapy from an accredited program.
• Active PTA license in state of practice.
• Commitment to continuing education and licensure maintenance.
• Ability to work under supervision of a Physical Therapist.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the PT direction.
• Assist in developing target lists, goals, and treatment plans.
• Conduct individual and group treatment focusing on strength, mobility, gait, balance, and ROM.
• Monitor residents’ response to intervention and modify as indicated.
• Assist with scheduling in coordination with the PT and team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Participate in resident, family, and staff education.
• Report to work on time and maintain productivity.
