Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility. You will work under the direct supervision of a Physical Therapist to deliver both individual and group therapy, focusing on mobility, strength, balance and functional goals.

Responsibilities include screenings, evaluations assistance, treatment planning, and CMS-compliant documentation in NetHealth EMR; commitment to ongoing licensure, and collaboration with the care team.

Qualifications

• Graduate PTA program from an accredited school.
• Active PTA license in the applicable state(s).
• Commitment to ongoing education and licensure maintenance.

Responsibilities

• Provide PT services under supervision of the PT.
• Conduct screenings to determine intervention needs.
• Assist in evaluations and treatment planning with the PT.
• Assist in setting short- and long-term goals and plans.
• Deliver individual and group therapy including mobility and strength work.
• Document progress and adjust plans with PT input.
• Coordinate schedules with the care team and CMS expectations in NetHealth.
