Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Norwich, NY. The PRN position supports ongoing therapy services under the supervision of a licensed Physical Therapist.

Responsibilities include resident screenings, treatment planning, and delivery of both individual and group PT sessions, with emphasis on strength, mobility, balance, gait, and activity tolerance. CMS documentation via NetHealth is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluations under supervision.
• Assist in developing treatment plans with short- and long-term goals under the Physical Therapist.
• Conduct individual and group treatment addressing strength, mobility, balance, gait, ROM, and modalities.
• Assist in selecting PT techniques/media and adapt as needed after consultation with the PT.
• Monitor residents' response and modify plan with PT guidance.
• Establish and modify resident schedules with input from PT and team.
• Adhere to schedules and adjust for treatment regimens.
• Train residents, family, and staff in equipment use.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in planning and evaluating therapy programs.
• Report to work on time and coordinate shifts for productivity.
• Assist with Quality Assurance activities as required.
• Complete compliant CMS documentation in NetHealth EMR systems.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license

Continuing education to maintain licensure
