Job summary

Evolve Therapy Services seeks a qualified Physical Therapy Assistant for an in-house Skilled Nursing Facility in Orange, NJ. This PRN position supports the Physical Therapist with therapy services, screenings, and treatment planning to ensure high-quality resident care.

Responsibilities include supervision by a PT, scheduling coordination, and CMS-compliant documentation in NetHealth EMR. Requires an active PTA license and an associate degree in physical therapy.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment planning under supervision of the Physical Therapist.
• Assist in establishing and modifying resident schedules in conjunction with the Physical Therapist and team.
• Complete compliant and medically necessary documentation in NetHealth EMR per CMS expectations.
