Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house skilled nursing facility in Orange, NJ. This PRN position supports the Physical Therapist and delivers both individual and group therapy to residents.

Responsibilities include screenings, treatment planning, progress monitoring, and staff education while maintaining CMS-compliant documentation in NetHealth EMR.

Competenze

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Mansioni

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list, including short-term goals and long-term goals and treatment plan.
• Conduct both individual and group treatment as assigned and outlined in the treatment plan.
• Assist in selecting PT techniques/media and adjust as indicated.
• Monitor residents’ response to intervention and consult with the PT for modifications.
• Assist in establishing and modifying resident schedules with the PT and team.
• Adhere to department and resident schedule and modify as needed.
• Train resident and staff in the use of equipment.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning and follow-up of therapy programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance and CMS documentation in NetHealth EMR.
