Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Norwich, NY. This PRN position supports the Physical Therapist with resident care, assessments, and treatment plans to promote mobility and independence.

You will implement therapy programs, document progress in NetHealth EMR, and collaborate with clinicians, residents, and families to ensure quality, compliant care and timely therapy services.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation by the Physical Therapist and in developing treatment plans.
• Implement individual and group treatment per the plan, including mobility, gait, ROM and modalities.
• Monitor residents' responses and modify interventions as directed by the Physical Therapist.
• Assist in establishing and modifying resident schedules with the team.
• Educate residents, families, and staff on use of therapy equipment.
• Ensure compliant, medically necessary documentation in CMS-compliant systems.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license
