Job summary

Evolve Therapy Services seeks a dedicated Physical Therapist to join our in-house Skilled Nursing Facility team in Altoona, PA. This position offers flexibility as Full Time, Part Time, or PRN, and focuses on delivering comprehensive PT services to residents in collaboration with an interdisciplinary team.

As part of the role, you will assess residents, develop treatment plans with short- and long-term goals, and ensure CMS-compliant documentation in NetHealth EMR.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop treatment plans with short and long-term goals and implement them.
• Conduct screenings and evaluations to determine need for intervention and treatment.
• Communicate evaluation findings and recommendations to residents, families and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
