Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to join our in-house SNF team in Webster, MA. The role reports to a Physical Therapist and focuses on delivering patient-centered care, under supervision, to support resident mobility and function.

You will participate in screenings, help set treatment goals, implement plans, and educate residents and families. A valid PTA license in Massachusetts and ongoing continuing education are required to maintain licensure.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in identifying target goals and treatment plans with the Physical Therapist.
• Conduct both individual and group treatment as outlined in the plan, including mobility, strength, gait, and modalities.
• Assist in selecting techniques/media and sequence activities with input from the Physical Therapist.
• Monitor resident response to intervention and consult the Physical Therapist for modifications.
• Help establish and modify resident schedules with the PT and team members.
• Adhere to schedules and modify as needed for treatment regimens.
• Train residents and families in equipment use; participate in education for residents, families, and staff.
• Maintain sound judgment in planning and follow-up of therapy programs.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.

Education
