Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. You will work under the supervision of a Physical Therapist to deliver patient-focused care and therapy plans that support resident mobility and quality of life.

Responsibilities include conducting screenings, assisting with goals and treatment plans, coordinating schedules with the care team, and ensuring timely documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying targets and goals within treatment plans.
• Conduct both individual and group treatments as outlined in the treatment plan.
• Monitor residents’ response to intervention and modify as indicated in consultation with the Physical Therapist.
• Establish and modify individual resident schedules with the Physical Therapist and team.

Education
