Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant to join our in-house skilled nursing facility partner in Webster, MA. You will work under the supervision of the Physical Therapist to deliver patient‑centered care.

Responsibilities include conducting screenings, implementing treatment plans, documenting CMS‑compliant notes in NetHealth EMR, and collaborating with residents, families and staff to promote mobility and independence.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Maintains licensure and pursues continuing education.
• Active Physical Therapy Assistant license in state(s) of practice.

Responsibilities

• Provide physical therapy services under supervision of PT.
• Conduct screenings to determine need for intervention.
• Assist in evaluation and treatment planning with PT.
• Implement treatment plans focusing on strength, mobility, balance and gait.
• Document CMS-compliant notes in NetHealth EMR and participate in QA.
• Educate residents, families and staff on therapies and equipment.
