Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. You will work under a Physical Therapist to deliver patient-centered therapy and participate in team care.

The role requires an Associate’s degree, an active PTA license in the practicing state(s), and ongoing continuing education to maintain licensure. Clinical documentation will be CMS-compliant in NetHealth EMR.

Qualifikationen

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Aufgaben

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in identifying target lists, short-term and long-term goals and treatment plans.
• Conduct individual and group treatments per the plan including strength, mobility, gait, ROM and modalities.
• Assist in selecting techniques/media and adapt as indicated by resident performance in consultation with the PT.
• Monitor resident response to intervention and modify as needed in consultation with the PT.
• Help establish resident schedules with the PT and team members.
• Adhere to schedules and treatment regimens, adjusting as needed.
• Train resident, family and staff in equipment use and safety.
• Participate in resident, family, and staff education.
• Ensure CMS-compliant documentation in NetHealth EMR.
