Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. This full-time role supports therapy under the supervision of a Physical Therapist and focuses on resident care quality.

You will conduct screenings, assist in evaluations, help establish treatment plans, and provide both individual and group therapies, including mobility and gait training. Qualifications include an associate degree and active PTA license.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine intervention needs.
• Assist in evaluations under the direction of the Physical Therapist.
• Assist in establishing target goals and treatment plans.
• Deliver individual and group treatment addressing strength, mobility, balance, gait, ROM, and modalities.
• Assist in selecting techniques and adapting activities per resident performance.
• Monitor resident responses and notify the PT of modifications.
• Help schedule residents with PT and other team members.
• Adhere to department and resident schedules and adjust as needed.
• Train residents, families, and staff in equipment use.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in planning and following through therapy programs.
• Report to work on time and maintain productivity during shifts.
• Assist in Quality Assurance activities and CMS-compliant documentation in NetHealth EMR.
