Job summary

Evolve Therapy Services in Webster, MA is seeking a dedicated Physical Therapy Assistant to work in an in-house Skilled Nursing Facility. The PTA will perform therapy under the supervision of a Physical Therapist, conduct resident screenings, help plan care, and support group and individual sessions focused on mobility, strength, balance and functional goals.

The role requires an active PTA license in the state, an accredited associate degree, and ongoing continuing education to maintain

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and develop treatment plans with the PT.
• Deliver individual and group therapies focusing on strength, mobility, balance and gait.
• Assist in selecting techniques/media and adapt as needed.
• Monitor resident response and modify plan with PT direction.
• Establish and modify resident schedules with the PT and team.
• Educate residents, families and staff on therapy use of equipment.
• Document in CMS-compliant NetHealth EMR accurately.
• Participate in QA activities and maintain scheduling efficiency.
