Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Newark, OH. You will work under the supervision of a Physical Therapist to deliver planned, patient-focused therapy and participate in team education.

Responsibilities include screenings, treatment planning, group and individual therapy, documentation in NetHealth, and contributing to quality assurance.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Ability to plan, organize and direct PT services.
• CMS documentation via NetHealth EMR.

Responsibilities

• Provide PT services to residents under supervision of PT.
• Conduct screenings to determine need for intervention.
• Assist in evaluations under PT direction.
• Assist in identifying goals and treatment plans.
• Conduct individual and group treatments covering mobility, balance, ROM, and modalities.
• Assist in selecting PT techniques and adapting activities.
• Monitor residents' response and consult PT for modifications.
• Help establish/modify resident schedules with PT and team.
• Adhere to department and resident schedules.
• Train residents and staff in using equipment and applications.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation and follow-up of therapies.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance activities as required.
• Complete CMS-compliant documentation in NetHealth.
