Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to join our in-house team at a Skilled Nursing Facility in Newark, OH. The PTA will work under the supervision of a Physical Therapist to deliver a range of therapies focused on resident mobility, strength, balance, and functional independence.

Responsibilities include screenings, assisting with evaluations, developing and following treatment plans, and educating residents and families.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations under the direction of the Physical Therapist.
• Assist in identifying targets and treatment plans with short-term and long-term goals.
• Conduct individual and group treatments including strength, mobility, balance, gait, and ROM work.

Education
