Job summary

Evolve Therapy Services seeks a dedicated Physical Therapy Assistant to join our in-house SNF team in Newark, OH. The role supports the Physical Therapist in delivering care to residents and implementing treatment plans.

You will conduct screenings, participate in group and individual sessions, and help document CMS-compliant therapy progress in NetHealth EMR. Collaboration with the PT and care team is essential.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure.
• CMS documentation requirements in NetHealth EMR systems.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing treatment plans with short-term and long-term goals.
• Conduct individual and group treatment addressing mobility, strength, balance, and ROM as outlined by the PT.
• Assist in selecting techniques/media and adapt as needed in consultation with the PT.
• Monitor residents’ response and modify as indicated by the PT.
• Coordinate schedules with PT and team members.
• Train residents, families, and staff in equipment use.
• Participate in education for residents, families, and staff.
• Maintain compliant and medically necessary documentation for CMS.
