Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Warren, Ohio. The PTA will provide services under the supervision of a Physical Therapist and help monitor patient progress throughout treatment plans.

The role requires an Associate's degree in Physical Therapy and an active PTA license, with ongoing continuing education to maintain licensure.

Qualifications

• Associate's degree required.
• Active Physical Therapy Assistant license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the PT.
• Conduct screenings to determine need for intervention/treatment.
• Assist PT in evaluating goals and treatment plans.
• Assist in establishing resident schedules with PT and team.
• Train residents and staff in use of equipment; contribute to education of families.
