Job summary

Evolve Therapy Services, LLC is hiring a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Newark, Ohio. The full-time role supports the Physical Therapist by providing patient care and contributing to treatment plans.

Responsibilities include resident screenings, individual and group therapies, documentation in NetHealth EMR, and collaboration with the care team to ensure quality resident services. Ongoing licensure and CEU maintenance are required.

Qualifications

• Associate's degree in physical therapy is required.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluations and in developing goals and treatment plans.
• Conduct both individual and group treatments as outlined in the treatment plan.
• Assist in selection and progression of therapy techniques and modalities.
• Document CMS-compliant and medically necessary progress notes in NetHealth EMR.
• Coordinate resident schedules with the Physical Therapist and care team.
• Educate residents, families and staff on therapy plans and equipment use.
