Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Newark, OH. You will provide physical therapy services under the supervision of a Physical Therapist to support resident mobility and functional goals.

The role requires an active Ohio PTA license and ongoing continuing education. Responsibilities include screenings, treatment planning, documentation in NetHealth EMR, and collaboration with the care team.

Qualifications

• Associate's degree in physical therapy.
• Active PTA license in state(s) of practice (Ohio).
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and development of treatment plans.
• Deliver individual and group therapy including mobility, strength, balance, gait, and functional activities.
• Document treatment and CMS-compliant records in NetHealth EMR.
• Coordinate schedules with the Physical Therapist and care team.
• Educate residents and families about therapy goals and progress.
