Job summary

Evolve Therapy Services seeks a licensed Physical Therapy Assistant to join our in-house Skilled Nursing Facility team in Newark, OH. You will work under the supervision of a Physical Therapist to deliver patient-centered therapy services.

Responsibilities include resident screenings, assisting with evaluations, developing treatment plans, and leading both individual and group sessions. Must maintain licensure and participate in ongoing education while supporting family and staff education.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Maintains ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under supervision.
• Conduct screenings to determine need for intervention.
• Assist in evaluation planning and treatment under the PT's direction.
• Develop and modify treatment plans and goals with the PT.
