Job summary

Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving our newly acquired AL/IL facility. We offer a supportive, family-like culture with opportunities for professional growth and meaningful impact on residents’ recovery and quality of life.

We provide a comprehensive compensation package including hourly pay of $34-$39, dependent on experience, plus benefits such as 401k, PTO, Medical, Dental, Vision, and licensure reimbursement after 1 year.

Qualifications

• Licensed Physical Therapy Assistant license required.
• Open to therapists at all experience levels.

Responsibilities

• Scheduling of residents for OP and IP services.
• Lead exercise group 3x weekly.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient's progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
