Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

Qualifications

• Licensed Physical Therapy Assistant (PTA) licensure required.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of a Physical Therapist.
• Provides skilled therapy services to patients in need of rehabilitation.
• Assesses and documents patient progress and communicates with the supervising PT to adjust treatments.
• Provides consultation to patients and families regarding physical disorders.
• Contributes to discharge summaries per regulatory requirements.
