Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facility. We value compassionate patient care and provide mentorship and growth opportunities.

Qualifications

Uses evidence-based practice to guide clinical interventions.
Provides skilled therapy under supervision of a Licensed Physical Therapist.
Delivers rehabilitative, skilled services to patients needing rehabilitation.
Assesses patient progress and communicates with supervising PT to adjust treatments.
Provides counseling to patients and families about physical disorders.
Assists in preparing discharge summaries and care plans as required.

Responsibilities

Utilizes evidence-based practice to support clinical interventions.
Provides skilled therapy under supervision of a Physical Therapist.
Provides skilled therapy services to patients who need rehabilitation services.
Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
Provides consultation and counseling to patients and families related to physical disorders.
Provides input for discharge summaries per regulatory requirements.
