Continuum Rehab Group in Albuquerque, NM is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving our newly acquired AL/IL facility. You will provide skilled, medically necessary treatment under the supervision of a Physical Therapist and help build rehab programs that enhance residents' recovery and quality of life.

Responsibilities

• Schedule residents for outpatient and inpatient services.
• Lead exercise group 3x weekly.
• Provide rehabilitative, skilled treatment per clinical guidelines under supervision.
• Deliver skilled therapy services for rehabilitation needs.
• Assess progress and adjust treatments with supervising PT.
• Counsel patients and families on physical disorders and care options.
• Support discharge planning documentation per regulations.

Experience/Qualifications

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some Duties For This Position Include

• Scheduling of residents for OP and IP services.
• Lead exercise group 3x weekly.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
