Qualifications

• Degree in Physical Therapy from a CAPTE-accredited program.
• Current license from the state Board of Physical Therapy Examiners.
• Minimum 3 years’ outpatient Physical Therapy experience.
• Current BLS certification.
• Experience using electronic medical records.

Responsibilities

• Collaborate with care team including physicians, PAs/NPs, and other therapists.
• Administer physical therapy treatments and educate patients.
• Develop and implement goal-directed treatment plans based on assessments.
• Coordinate care and ensure proper follow-up and referrals as needed.
• Teach home exercises and document care in EMR.
• Maintain safe, clean work environment and infection-control practices.
• Participate in continuing education and staff development.
