Job summary

JAG Physical Therapy in New York is seeking a licensed Physical Therapist to assess patients, develop treatment plans, and deliver evidence-based therapies. The role emphasizes high-energy, positive environments and strong confidentiality.

Qualifications

• Bachelor’s or Master’s Degree required, Doctorate degree preferred
• Licensed Physical Therapist in the state practicing in
• Completed state regulated CEU requirement in NY/NJ
• Experience/knowledge with an EMR software is preferred
• Excellent oral, written communication
• Strong clinical decision-making skills
• Excellent work ethic and dedication to patient success
• Maintains a positive, professional demeanor

Responsibilities

• Collect and record patient care data following format and guidelines reflecting reimbursement impact
• Perform initial evaluations, daily notes, progress reports and discharge plans
• Record daily billing and meet productivity guidelines
• Prepare for evaluation per department policies and standards
• Select and administer tests and measures
• Formulate diagnosis, goals, prognosis and plan of care
• Establish treatment plan and collaborate with interdisciplinary team
• Identify need for consultations and discharge planning
• Provide education and training to patients and support system
• Oversee PTA, ATC, and PT aides
• Attend billing and coding seminars
• Promote JAG in the community
