JAG Physical Therapy is seeking a licensed Physical Therapist for our Brooklyn (Midwood-Ocean Ave) location. The role involves assessing patients, building creative treatment plans, and guiding care with a patient-centered approach.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate preferred.
• Licensed Physical Therapist in the state practicing in NY/NJ.
• Completed state CEU requirement for NY/NJ.
• Experience with EMR software preferred.
• Excellent oral and written communication skills.
• Strong clinical decision-making and patient-focused approach.

Responsibilities

• Collects and records patient care data per department guidelines.
• Performs initial evaluation, daily notes, progress reports, and discharge planning.
• Prepare evaluations with patient history and diagnoses.
• Establishes treatment plans and collaborates with the team.
• Administers musculoskeletal interventions and therapeutic activities.
• Oversees PTA, ATC, and PT aides; participates in billing and coding training.
