Job summary

Mercy Health Center is seeking a Physical Therapist to evaluate, develop, and implement individualized treatment plans for adult and geriatric patients in a collaborative care setting. You will supervise rehab staff and coordinate with an interdisciplinary team to deliver high-quality care.

Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in the state of practice.
• Current BLS (Basic Life Support) certification.
• Six months to one year of physical therapy experience preferred.
• Experience with adult and geriatric populations preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized plans of care.
• Deliver evidence-based PT interventions and monitor progress.
• Educate patients and caregivers on exercises and home programs.
• Maintain accurate documentation and ensure compliance with standards.
• Coordinate care with physicians and interdisciplinary team members.
