Qualifications

Responsibilities

• Evaluate patients and develop individualized treatment plans based on referrals.
• Provide interventions to improve mobility, strength, balance, and function.
• Monitor progress and document evaluations, treatments, and outcomes.
• Educate patients and families on plans, home programs, and discharge.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Maintain compliant clinical documentation and safety standards.
